How to Recognize Drug Addiction: Signs of Substance Use

Someone using drugs may appear and behave normally, especially in the early stages. However, certain physical signs and behaviors can reveal the presence or absence of addiction.

Physiological signs of drug use include:

  • Change in skin color. The face becomes pale — sometimes noticeably so. In some cases, the skin on the face and upper body may appear flushed instead.
  • Impaired motor coordination. Movements become abrupt and unsteady. The person may frequently lose balance or sway while walking. Handwriting deteriorates significantly — even holding a pen may become difficult.
  • Unusual shine in the eyes and abnormal pupil behavior — pupils may constrict to tiny points or dilate significantly. Under the influence, pupils often stop reacting to light altogether.
  • Changes in activity levels. The person may first appear hyperactive, then quickly become lethargic and apathetic. Drug users often seek solitude, curling up in hidden places to avoid attention.
  • Changes in voice. It may become hoarse, with frequent throat clearing. The mouth often feels dry, although in some cases, excessive salivation may occur instead.

Psychological signs of drug use often include:

  • Frequent mood swings. The person may seem overly cheerful, then suddenly aggressive or irritable. Speech patterns change — becoming either overly precise or completely incoherent.
  • Altered consciousness. Thought processes become fragmented, as if the mind is filled with random memories or emotions. The person may withdraw and become unresponsive to external stimuli.

First Signs of Different Drug Types

Families seeking help at our center often ask what kind of substances their loved one might be using. Only a certified addiction specialist can give a definitive answer after an evaluation and lab testing, but it’s useful to understand what specific drugs may cause certain symptoms. Here’s how to recognize addiction based on the type of substance.

Opiates

People using opiates often experience extreme drowsiness. For example, they may fall asleep mid-conversation or drop a cigarette as they begin to nod off.

They also tend to speak very slowly, sometimes repeating the same phrase multiple times. As long as they aren’t disturbed, they often appear calm and friendly.

One key physiological sign is pinpoint pupils. Other indicators include a lack of pain sensitivity and disrupted sleep patterns. These individuals often stay up late and sleep through the day, leaving their room only to seek the next dose.

Cannabis

Early signs include red eyes and dilated pupils. The mouth feels very dry. Movements may be erratic, with constant fidgeting or changing posture. After the high fades, a sudden, intense appetite often appears — a classic indicator of marijuana use.

Sedatives

The effects mimic alcohol intoxication. Speech becomes slurred and unclear, motor coordination is impaired. In high doses, sedatives can cause hallucinations.

Stimulants

These substances trigger hyperactivity and exaggerated responsiveness. The person can’t sit still and often paces or fidgets rapidly. Speech becomes erratic, jumping quickly from one topic to another. Their pupils are typically dilated. Some types of stimulants also increase sexual arousal and are commonly used in pairs.

Inhalants

These are most commonly abused by teenagers. Inhalant use mimics drunkenness — loud reactions, provocative behavior, excessive laughter, or aggression. The first sign is often the smell of gasoline, glue, or acetone. Hallucinations are common and are often described as “cartoons” by the users themselves.

Hallucinogens

A telltale sign is bizarre behavior — seeing animals or hearing phantom sounds. The person may talk to the TV or a glass display case. They appear irrational while under the influence, and once the drug wears off, they may experience deep depression. Long-term use often leads to psychosis.
